All Packages Include:
- Removal from the place of death.
- Transportation to a contract mortuary.*
- Necessary care and handling of the deceased.
- One death certificate.
- FCAO record fee.
- State filing fees.**
- Preparation of all required permits.
- Claims-notification-benefits assistance.
- Delivery to a local crematory or cemetery.
- Return of cremated remains to survivors locally. (Cremation only)
* Transportation: Removal from place of death or disposition of remains
within a 25 mile radius of the mortuary is included in all packages. Additional
mileage is $1.00 per mile overall. A $75.00 additional charge will be assessed if
the removal of the deceased takes place outside of normal business hours, of
8:00 AM to 5:00 PM, Monday through Friday.
** The $20.00 Oregon State Filing Fee will not be charged for deaths occurring outside of the State of Oregon. A $75.00 courier fee for filing the burial/transit permit will be added if the death occurs in Washington State.

Additional Information:
- Members wishing a different casket are allowed a $300 credit toward purchase of a traditional casket or a $75 credit toward purchase of a cremation container when the package unit is not used. Prices for these substitutions will be taken from the mortuary general price list with no further discount.
- With the exception of caskets or containers (above), members will receive a 20% discount from the FTC general price list for merchandise or services ancillary to, but not an enhancement of, a package. Members can also receive a 20% discount for any services or merchandise purchased separately from the packages listed above.
- There is a discount for association members wishing to purchase property, merchandise or services from any of our contract cemeteries in Oregon: 15% off property purchased pre-need, 15% off merchandise or services pre-need, and 10% off all purchases at time of need.
- The term “Basic Services of Funeral Director and Staff” is defined as: “Personnel available 24 hours a day, 365 days a year to respond to initial call; arrangement conference; coordinating service plans with cemetery, crematory, and/or other parties involved in the final disposition of the deceased; clerical assistance in the completion of various forms associated with a funeral; securing and recording the death certificate and disposition permit; overhead, such as facility maintenance, equipment and inventory costs; insurance and administration expenses, and governmental compliance”.
